What to expect during your physical
Our provider is an approved medical examiner through the National Registry of Certified Medical Examiners administered by the Federal Motor Carrier Safety Administration.
Under federal law, operators of commercial motor vehicles are required to undergo a physical examination, drug testing, and alcohol testing once every two years, in addition to random substance abuse testing. All these services are offered at Elite Screening and Wellness or conveniently at your place of employment.

After your evaluation, your provider will accurately complete the Medical Examination Report for Commercial Driver Fitness Determination form, provide a summary of your exam and a Medical Examiner’s Certificate as appropriate.
We aim to provide affordable DOT medical examinations / physicals at your convenience; however, insurance is not accepted for physicals, and payment is due at the time of visit.
